First, let's start out with what I like about this image. I like that it makes me think about the two-headed Roman God Janus who is always looking forward and backward. I also like that the background circle is not perfectly circular because it does look like an old hand-minted coin.

Second, things to improve on. The texture of the image is too smooth, like a pastel drawing on a chalk board. The smoothness is appropriate for the baby face, but elderly face could use a bit more roughing up. Since you use Painter I'd try using the Papers palette and punching up the grain of the paper, just to add a little grit to the image. I'd also play with the contrast/brightness settings a bit to deepen the shadows.

Overall, it's a promising piece with great potential and I'd love to see a re-vamped version.
